Financial Affidavit

I need to get divorced as soon as possible.  My wife and I have a lot of valuables and assets that we need to divide up.  I called the Broward county clerk of courts and they told me that I would need to file a family law financial affidavit to split up the things between me and my soon to be ex.  Do you know where I can download one that I can fill out online for free?
