Dear Carol, There are new financial affidavit forms as of September 2012. We do not yet have them posted on our site. They are available on the Florida Supreme Court site - www.flcourts.org. The "long" form is for individuals with incomes above $50,000. per year; and the "short" form is for individuals earning less than $50,000. per year.
