There are rules, and general formats for a medical negligence complaint, but I don't know that there is a specific form for that. Attorneys usually handle this type of suit, personal injury, because there may be damages that you would likely be unaware of. Medical negligence, personal injury, and torts are extremely complex areas of the law, and usually not suitable to pursue pro se.
