Format your Motion/ Request to Withdraw just as all the other paperwork in your case, with the same information at the top - identifying the judicial circuit, county, parties, and case number. Then state your reasons for your request. Then finish it just as all other documents in your case, with your signature above the address block; and the certification at the end, where you state you are sending this document to the other party.
File the original with the clerk of court, mail a copy to the other party.
