We looked at the form on the clerk's website, and it does not say that the Petitioner must file it. It only says that your dissolution must be uncontested. Here is a link to the form:
My suggestion is that you complete it and file it. Or, alternatively - if the clerk's will not accept it, go visit your soon to be ex, with a form filled out with her information, ask her to sign it, and file it.
