Declaration of Domicile forms are available at many of the clerk of court's websites, although I didn't find one for Duval County. The forms are nearly identical throughout the state, and so you could likely use a form from a different county and conform it to your county. Also the clerk of court's office at the courthouse should be able to provide you with a blank form. We will soon be posting a form on our site as a free download. A Declaration of Domicile form, in addition to being used for your purpose, is also sometimes used to establish residency for homestead exemption.

Dear Joe,
The Duval County Clerk of court should have this form, although I could not find it on their site. A Declaration of Domicile, among other uses, is needed by some people to prove their residence to establish their Homestead Exemption.
