The format for your motion is similar to any other motion or pleading in your case. All of the information at the top of the page stays the same - case number, name of the county and circuit, parties names, etc. Title your motion as Motion for Change of Venue, then in the first line of your motion identify your role in the law suit, whether you are the defendant or plaintiff; petitioner or respondent. Next, in numbered paragraphs state why you need to transfer jurisdiction. One reason you might need to transfer venue is that the court is too far away, and is inconvenient. Make one point for each numbered paragraph. Then at the bottom put a line for your signature and your address underneath that. Then add the certification, which is the part that says you are sending this motion to the other party, and add another signature line. Go to your clerk of court website and find a motion to use as a sample, so that you can replicate the format.
